A Case of Premenstrual Asthma in Aspirin-sensitive Asthma

Abstract
Exacerbation of asthma symptoms just before and during menses has been recognized in women with asthma and confirmed by report that peak expiratory flow rate decreases during the premenstrual and menstrual period in one third of women. In
general,
the
fall in the peak expiratory flow rate is usually modest. However, worsening of asthma in relation to menstruation can be frequently quite severe and has been described as a cause of repeated hospitalizations, recurrent repiratory failure, and
death.
We experienced a case of premenstrual asthma in an aspirin-sensitive asthmatic patient which was confirem by lysine-aspirin bronchoprovocation test. This patient was 49-year-old women with recurrent life-threatening premenstrual excerbations of
asthma
for 8 years who did not respond to conventional treatment including high-dose corticosteroid.
Also, there was a significant fall of peak flow rate before her menstrual period. The addition of intramuscular progesterone(100mg, two times/week) to the anti-asthmatic regimen slightly lessened the premenstrual dips in peak flow, and daily
requirements of prednisolone could be reduced.
